Understanding blood values: which values your medications affect

This guide flips the logic around: not "which value means which illness?", but "which medication needs which check — and how often?". That way you understand why your team is keeping an eye on exactly these values.

1. Why medications are monitored in the first place

No medication acts only where it should. Many active substances are processed by the liver or excreted via the kidneys — and both organs can come under strain in the process without you noticing anything at first. Other medications interfere with the salt balance (such as potassium and sodium) or affect the blood count. That is precisely why there are check-up appointments: they make silent changes visible before they become a problem.

The good news: in the vast majority of cases these checks are pure routine and the values stay in the green zone. They are not a sign that your medication is "dangerous" — they are the safety net that lets you take it with peace of mind in the first place. A slightly deviating value also rarely means that something has to happen immediately; often a somewhat closer follow-up check is all that is needed.

Important to understand: reference ranges on the lab report are statistical spans, not hard boundaries between "healthy" and "ill". What a value means for you depends on your history, your medications and how things develop over time. That is why only your treating practice can correctly interpret a value — not a comparison with a table on the internet.


2. The key table: medication, monitored value and interval

3. Methotrexate: keeping an eye on blood count and liver

Methotrexate (MTX) is used in low doses for rheumatic conditions and psoriasis — usually as a single dose once a week. Because it slows cell division, it can alter the blood count (white and red blood cells, platelets) and cause the liver values to rise. Kidney function also plays a role, because MTX is excreted via the kidneys.

That is why monitoring is close at the start of therapy and after each dose increase — often every two to four weeks initially. If treatment runs stably, the intervals become longer, usually to every eight to twelve weeks. If a value stands out, it does not automatically mean that MTX will be stopped: often the dose is simply adjusted, a shorter-term follow-up is done, or the accompanying folic acid is reviewed. A classic beginner's mistake is confusing the weekly intake with a daily one — with MTX this can become dangerous, which is why the checks are doubly important.


4. Statins: liver values and CK only with muscle pain

Statins such as Atorvastatin lower LDL cholesterol and are processed by the liver. That is why the liver values are determined before starting therapy. Contrary to earlier assumptions, close ongoing monitoring of the liver is no longer necessary for most people today — routine re-testing tends to be restrained, but targeted in the event of symptoms or a dose change.

A second value only becomes important when needed: creatine kinase (CK). It rises when muscle cells are under strain. In rare cases statins can cause muscle complaints — from a harmless sensation of aching muscles to, very rarely, serious muscle breakdown. So the rule is: actively report new, unexplained muscle pain or muscle weakness to your practice, especially together with dark urine. Then the CK is measured and interpreted. Without such complaints, a routine CK check is generally not required.


5. Levothyroxine: everything revolves around the TSH

Levothyroxine replaces the thyroid hormone when the thyroid produces too little. The decisive monitored value is the TSH — a control hormone from the pituitary gland that indirectly shows whether the dose is right. A high TSH usually indicates an undersupply (dose too low), a low TSH an oversupply (dose too high).

After each dose change it takes a few weeks for the value to settle — which is why the TSH is re-measured at the earliest after about six to eight weeks. Once the adjustment is stable, one check per year is usually enough. Important for reliability: Levothyroxine is taken on an empty stomach, and the blood draw should take place under comparable conditions where possible. In special situations — such as during pregnancy — different target values and shorter intervals apply; this too is determined by the practice.

6. Allopurinol: uric acid and kidney values

Allopurinol lowers uric acid and thereby prevents gout attacks. The most important monitored value is therefore uric acid itself: it shows whether the dose is enough to reach a target range. If the target value is not reached, the dose can be increased step by step — the rhythm depends on how things develop.

In addition, the kidney values are watched, because kidney function influences how Allopurinol may be dosed. Especially at the start, monitoring is therefore often closer and the dose is cautiously eased in. One more point: in the first few weeks the lowering of uric acid can even briefly trigger a gout attack — this is known and no reason to stop the medication on your own. Here too, the rule is to leave the further management to the practice.


7. Diuretics: potassium, sodium and kidney

Water-eliminating medications (diuretics) such as Furosemide flush out water — and with the water, salts too. That is why with them the focus is above all on potassium and sodium as well as the kidney values. Potassium that is too low can promote cardiac arrhythmias, sodium that is too low can cause confusion and weakness, among other things.

Monitoring is typically done after starting and after each dose change, then at regular intervals — especially if other cardiovascular medications are also being taken or an illness is added. Everyday situations count too: with heavy sweating, diarrhoea or a gastrointestinal infection, the salt balance can tip more quickly. Report such phases, then a check can be done earlier. Some diuretics, by the way, work in the opposite way and cause potassium to rise — this too is why individual assessment is important.


8. DOACs and ACE inhibitors: kidney function has a say

Modern blood thinners (DOACs) are excreted largely via the kidneys. If kidney function deteriorates, the active substance can accumulate in the body and the bleeding risk can rise. That is why the kidney is checked before starting and regularly afterwards — usually at least yearly, more often with impaired function. Unlike with the older anticoagulants (phenprocoumon/Marcumar), DOACs require no routine coagulation measurement.

ACE inhibitors lower blood pressure and protect the kidney, but especially at the start they can temporarily change the kidney values and cause the potassium to rise. A slight rise in the kidney values after starting therapy is often expected and not an alarm signal — the practice therefore checks specifically a few weeks after starting. It becomes critical above all in combination with other medications or with fluid deficiency, which is why a check is especially worthwhile here.


9. Your checklist for blood checks

  • Keep check-up appointments — they are the safety net of your therapy, especially when you feel well.
  • Bring a complete medication list to the blood draw — over-the-counter products also affect some values.
  • Report symptoms — such as muscle pain on statins or weakness on diuretics, rather than waiting.
  • Don't interpret values yourself and don't change any dose on your own — the interpretation belongs in the practice.
