Sildenafil belongs to the group of PDE-5 inhibitors, named after the enzyme phosphodiesterase 5, which the active ingredient blocks. Originally sildenafil was developed for the treatment of angina pectoris, that is a circulation disorder at the heart. In the studies the known effect on the erection then showed, and since 1998 it has been on the market as Viagra. Today the active ingredient is used above all with erectile dysfunction, alongside also with a certain form of pulmonary hypertension.
To understand the effect, a look at the natural erection helps. With sexual arousal a messenger called cGMP is formed in the penis, which relaxes the blood vessels so that more blood can flow in and an erection arises. An enzyme, the phosphodiesterase 5, breaks this messenger down again. Exactly here is where sildenafil comes in: it inhibits this enzyme, so that the messenger is preserved longer and the blood flow is improved. Very important here is a point that is often misunderstood: sildenafil only works erection-enhancing, not erection-producing. Without sexual arousal nothing happens. The active ingredient therefore only creates the preconditions for a better blood flow; the decisive impulse must come from the natural arousal. This also explains why some people are disappointed when the hoped-for effect fails to appear: often there is simply a lack of sufficient stimulation or of a relaxed situation.
Not a desire agent but a circulation aid
A common misunderstanding is that sildenafil is an aphrodisiac that creates desire or triggers an erection by itself. That is wrong. The active ingredient neither increases the sexual desire nor works without arousal. It only improves the physical reaction when a sexual stimulation is present. Anyone who feels no arousal or has other causes for the problems is not helped by sildenafil alone. All the more important is the medical clarification of the actual cause, which can at the same time be the most effective help.
Sildenafil is as a rule taken on demand, that is specifically before a planned sexual activity. Usual is the intake about 30 to 60 minutes beforehand with a glass of water. The effect then sets in only in connection with sexual arousal and mostly lasts around four to six hours. This does not mean a continuous erection over hours but a time window in which the natural reaction to arousal is facilitated. Per 24 hours the agent should only be used once.
There are some practical points that influence the onset of effect. A heavy, fatty meal shortly before intake can delay the effect because the active ingredient is then absorbed more slowly. Alcohol too is unfavourable: it can weaken the effect and at the same time strengthen the blood-pressure-lowering effect, which favours dizziness. Anyone who wants to use it relaxed and safely is therefore better off avoiding a heavy meal and larger amounts of alcohol before intake. Sildenafil is usually available in the strengths 25, 50 and 100 milligrams, whereby it is often started with a medium dose and adjusted depending on effect and tolerability. There is a safety rule with sildenafil that is so important that it must come first: sildenafil must never be taken together with nitrates. Nitrates are medicines that are used with heart diseases such as angina pectoris, that is with a narrowing of the coronary vessels. Typical examples are nitro spray or nitro tablets that are given under the tongue, as well as longer-acting nitrates in tablet form. They widen the vessels and lower the blood pressure.
The problem is that sildenafil and nitrates work via the same biological pathway and strengthen each other massively in their blood-pressure-lowering effect. In combination the blood pressure can fall so strongly and suddenly that a circulatory collapse occurs. In the worst case a life-threatening shock, a heart attack or a stroke threaten. Exactly for this reason this combination is absolutely forbidden, and exactly for this reason the medical clarification before the first intake is so important, because many heart patients take nitrates. In addition, after taking sildenafil, no emergency nitrate may be given for a certain period either, for example if heart complaints occur in the meantime. In an emergency the treatment team must therefore know that sildenafil was recently taken so that it does not accidentally administer nitrates.
Never combine sildenafil with nitrates or poppers
Never take sildenafil together with nitrates, not even with an emergency nitro spray for heart complaints. The same applies to so-called poppers, that is party drugs that contain nitrites and are used in a sexual context. This combination too can lead to a fatal drop in blood pressure. If you have been prescribed nitrates or use poppers, tell your medical practice this without fail before sildenafil even comes into question. In doubt it applies: no sildenafil without medical clearance.
Beyond the nitrates too there are important points around heart and circulation. Because sildenafil itself lowers the blood pressure slightly, caution is required with people who take blood pressure medicines. With a single, well-controlled blood pressure agent, the use is often possible, but with several agents or with so-called alpha blockers, the blood-pressure-lowering effect can strengthen and lead to dizziness or fainting. Here a time gap between the intakes can sometimes help, which must however be clarified medically.
A further important aspect is the physical strain. Sex is an exertion for heart and circulation. With certain severe cardiovascular diseases this strain can itself represent a risk, independently of the medicine. Also for this reason a medical examination belongs before the first intake, in which heart, blood pressure, previous illnesses and all taken medicines are discussed. Besides this, grapefruit and grapefruit juice can inhibit the breakdown of sildenafil and strengthen the effect, which is why they should be avoided. Certain medicines against fungal infections, HIV or other diseases also influence the active ingredient level.

Like every effective medicine, sildenafil too has side effects that mostly follow directly from the vessel-widening effect. Common are headache, a facial flushing, a blocked nose, digestive complaints and dizziness. These complaints are as a rule mild and temporary. Temporary visual disturbances can also occur, for example a slight blue tinge in the field of vision or an increased light sensitivity that subsides after a few hours.
There are, however, also rare, serious side effects one should know. A medical emergency is a painful prolonged erection that lasts longer than four hours and does not depend on sexual arousal, technically called priapism. It must be treated medically immediately, because otherwise the tissue can be damaged. Also to be taken seriously are a sudden loss of vision in one or both eyes or a sudden loss of hearing. In these cases the agent should be stopped immediately and medical help sought without delay. These serious events are rare, but they show why a medical accompaniment and an honest account of previous illnesses are so important. Emergency: erection longer than four hours
An erection that lasts longer than four hours and is painful is a medical emergency, even if the topic may be uncomfortable. In such situations quick action is more important than any shame.
Sildenafil is not the only PDE-5 inhibitor. A common comparison is the one with tadalafil. Both work according to the same principle but differ above all in the duration of effect. Sildenafil works about four to six hours and is taken specifically before the activity. Tadalafil works clearly longer, up to about 36 hours, which allows more spontaneity, and is additionally approved in a low dose for daily intake. Which active ingredient fits better depends on the personal needs, the tolerability and possible previous illnesses and should be decided together with the medical practice.
A final, important point: an erectile disorder is often more than an isolated problem. Behind it can be treatable diseases, for example diabetes, high blood pressure, a vascular calcification or mental strains such as stress and anxieties. Sometimes an erectile disorder is even an early warning signal for a cardiovascular disease. Therefore it is sensible to have the cause clarified medically instead of only covering up the symptom with a tablet. The vessels in the penis are particularly fine, which is why a beginning vascular calcification often makes itself noticeable there earlier than at the heart.
