1. What is low blood pressure?

Low blood pressure (medically hypotension) is generally defined as readings below about 100/60 mmHg. Unlike with high blood pressure, however, this threshold is mainly a guide — what matters is not the number alone, but whether the low readings cause you symptoms.

Many people naturally have low readings and notice nothing at all. Others feel tired, have trouble concentrating, get cold hands and feet, or become dizzy when standing up quickly. It is precisely this dizziness on standing — known as orthostatic hypotension — that is the most common and most typical symptom.

The most important point first Low blood pressure is usually harmless in itself and often even beneficial for the heart and blood vessels. It only needs treatment if it causes you symptoms or drops suddenly and sharply. The most commonly overlooked cause is medications.

2. Harmless or in need of treatment?

With low blood pressure, a change of perspective is worthwhile: while high blood pressure damages the cardiovascular system over time, low blood pressure is considered rather protective — statistically, people with low readings have a lower risk of heart attack and stroke. A low reading on its own is therefore no cause for concern and not an illness.

Two situations should be kept apart:

  • Persistently low readings without symptoms: harmless, no need for treatment. Anyone who feels fit does not need to do anything.
  • Low readings with symptoms: dizziness, light-headedness, visual disturbances or near-fainting — here it is worth looking for the cause and taking countermeasures, above all to avoid falls.

It becomes critical when blood pressure drops suddenly and sharply — for example after a major loss of fluids, bleeding or a severe infection. This can lead to circulatory collapse or even shock (see the chapter "When to see a doctor").


3. Causes & triggers

Low blood pressure is often simply a matter of predisposition — especially in young, slim women. Alongside this, however, there are a number of triggers that can be readily identified and, in some cases, influenced.

  • Medications (very often overlooked): blood-pressure-lowering drugs such as Ramipril, diuretics such as Torasemide, certain psychotropic drugs (e.g. some antidepressants) as well as prostate and Parkinson's medications can lower blood pressure considerably.
  • Lack of fluids: not drinking enough, sweating a lot, diarrhoea or vomiting — the circulation lacks volume.
  • Prolonged standing and heat: the blood "pools" in the legs, and this is made worse in summer.
  • Standing up quickly (orthostasis): the circulation cannot keep up quickly enough.
  • Hormonal causes: such as an underactive thyroid or adrenal insufficiency.
  • Heart conditions: less common — e.g. heart failure or arrhythmias.

The link with new or changed medications in particular often goes unnoticed. If symptoms began after a change of medication, this is an important clue for the conversation with your doctor — but please never stop taking medications on your own initiative.

4. Checking blood pressure & orthostasis yourself

  • Measure at rest: after sitting for 5 minutes, cuff at heart height, no talking.
  • Orthostasis test: first measure while lying down, then straight after standing up and again after 1–3 minutes of standing.
  • A red flag is: a drop in the upper value of more than 20 mmHg (or the lower value by more than 10 mmHg) on standing — often with dizziness.
  • Note it down: record readings, time, symptoms and current medications — ideal for the conversation with your doctor.

A single low reading without symptoms is no cause for concern. What matters is the overall picture of readings and symptoms over several days.

5. When to see a doctor?

Low blood pressure is usually harmless, but there are situations in which you should have it checked out or act immediately.

You should have it checked by a doctor promptly if the low blood pressure repeatedly causes you symptoms (dizziness, light-headedness, visual disturbances), if it has made you nearly or completely faint, or if the symptoms began after a change of medication. Newly occurring low blood pressure in older age should also be checked.

Call the emergency number 112 immediately In cases of very low blood pressure with confusion, cold clammy or pale skin, a very rapid pulse, after a major loss of fluids or bleeding — or with a faint accompanied by injury. These signs can indicate circulatory shock and are an emergency.

6. Diagnosis

Often the cause becomes clear simply from looking at the medication list and a simple measurement lying down and standing. A well-kept blood pressure and symptom log makes it even easier to make the connection.


7. What you can do yourself

  • Stand up slowly: from lying down, first sit up briefly, then stand — this prevents dizziness. "Pumping" the legs before standing up helps the circulation.
  • Drink enough: about 1.5–2 litres a day (unless otherwise advised by your doctor), more in hot weather. A glass of water stabilises things in the short term.
  • Exercise: regular activity such as walking, swimming or cycling trains the blood vessels and the circulation.
  • Don't keep salt too low: unlike with high blood pressure, salt intake here may be normal — but discuss this with the practice.
  • Alternating showers and compression stockings can further stimulate the circulation or reduce the pooling of blood.
  • Have your medications reviewed: if symptoms began after a change, speak with the practice — never stop taking them on your own initiative.
