1. What is paleness?

Paleness means that the skin and mucous membranes look lighter than usual — the healthy, rosy colour is missing. This is best seen not on the cheek, which is influenced by sun, make-up and skin type, but at sites with thin, well-perfused mucous membrane: the lips, the inside of the lower eyelids, the gums and the nail beds. If these areas look noticeably wan rather than rosy, that is a more reliable sign.

The rosy colour comes from the red blood in the small vessels just beneath the surface. Paleness therefore has essentially two causes: either there is less blood in the skin (the vessels are narrowed, for example in cold or circulatory weakness), or the blood itself contains less red blood pigment (haemoglobin), as in anaemia. Both routes lead to the same pale appearance — but the causes are very different.

The most important point first Many people are naturally pale — that alone is not a sign of illness. You should pay attention if the paleness has newly appeared or is increasing and is accompanied by fatigue, shortness of breath on exertion or a racing heart. This combination can point to anaemia.

2. Harmless or dangerous?

The decisive question is not "am I pale?", but "is the paleness new and what comes with it?". Two situations should be told apart:

  • Constant, constitutional paleness: usually harmless. Anyone who has always had a fair complexion, feels fit and has no accompanying symptoms generally has no reason to worry. Brief paleness from cold, fright or a circulatory dip is also normal.
  • Newly appeared or increasing paleness with symptoms: can be a sign of iron deficiency anaemia or another form of anaemia — especially together with fatigue, shortness of breath on exertion and a racing heart. This should be checked by a doctor.

It also becomes serious when paleness occurs suddenly and markedly — for example with cold sweats, circulatory collapse, chest pain, breathlessness or after bleeding. Then prompt action is needed (see the chapter "When to see a doctor").


3. Causes & triggers

Paleness is a symptom, not a diagnosis. It can have harmless everyday reasons, but it can also point to a treatable condition. The most common causes at a glance:

  • Anaemia: the most important medical cause. Usually an iron deficiency lies behind it — for example through heavy periods, an iron-poor diet or unnoticed blood loss. Less haemoglobin means paler skin and mucous membranes.
  • Low blood pressure and circulatory weakness: during circulatory dips the skin vessels narrow and the face turns wan — often together with dizziness and blacking out before the eyes.
  • Cold, fright and stress: the body draws blood away from the skin. This paleness is temporary and harmless.
  • Thyroid disorders: an underactive thyroid can be accompanied by pale, cool skin, fatigue and feeling cold.
  • Chronic conditions: for example kidney disease or long-standing inflammation can cause anaemia and thus paleness.
  • Acute blood loss: heavier bleeding — visible or internal — quickly leads to marked paleness and is an emergency.
  • Rare causes: among others, certain blood disorders; these are rare, but a reason not to ignore persistent paleness.

The link with fatigue, breathlessness on exertion or your period in particular can be observed well yourself. A simple record helps to spot patterns and to give the practice the right clues.

4. Self-check: what to look out for

Paleness cannot be measured at home, but it can be observed well. A few simple observations help you and the practice to interpret it correctly.

  • Look at the mucous membranes: gently pull the lower eyelid down — the inside should be a strong rosy colour. A very pale, almost white inner eyelid can point to anaemia.
  • Nail-bed test: press briefly on a fingernail and let go. If the colour returns only slowly and the nail bed looks pale, make a note of it.
  • Record accompanying symptoms: fatigue, breathlessness when climbing stairs, a racing heart, dizziness, headaches or trouble concentrating? These are important clues.
  • Note the course: how long has the paleness been present? Is it increasing? Are there heavy periods or other blood losses? These details help with the assessment.

Newly appeared, increasing paleness with fatigue and breathlessness should be checked by a doctor. Constant paleness without any symptoms, on the other hand, is usually harmless.

5. When to see a doctor?

Paleness is often harmless, but there are clear situations in which you should have it checked or act immediately.

You should have it checked by a doctor promptly if the paleness has newly appeared or is increasing and is accompanied by fatigue, shortness of breath on exertion, a racing heart, dizziness or a drop in performance. A check is also sensible with heavy periods, visible blood in the stool or unexplained weight loss — here anaemia can be clarified with a simple blood test.

Call the emergency number 112 immediately If sudden, marked paleness occurs together with cold sweats, circulatory collapse, chest pain, breathlessness or after heavier bleeding. This combination can point to threatening blood loss, shock or a heart problem and is an emergency.

6. Diagnosis & blood values

To find the cause of the paleness, the practice has simple and informative tests available. Usually a conversation, a brief examination and a blood sample are enough.

7. What you can do yourself

  • Observe accompanying symptoms: note fatigue, breathlessness, a racing heart and dizziness — they help to tell harmless paleness from paleness that needs checking.
  • Pay attention to an iron-rich diet: pulses, wholegrains, green vegetables and (if needed) meat provide iron; vitamin C improves absorption. Better not to have coffee and tea directly with an iron-rich meal.
  • Don't take iron on suspicion: only take iron supplements after a confirmed deficiency and in consultation with the practice — too much iron can be harmful.
