Never stop or pause Levetiracetam on your own. Abrupt discontinuation can trigger severe seizures, even status epilepticus – always agree any changes with your practice. Levetiracetam (brand name Keppra®) is a modern anti-epileptic drug used to prevent epileptic seizures. It is easy to titrate up, works quickly and needs no routine blood-level monitoring – which is why it is now one of the most frequently prescribed seizure medications.

During an epileptic seizure, nerve cells in the brain discharge suddenly, excessively and in sync. Levetiracetam binds to a particular protein in the nerve endings, the synaptic vesicle protein 2A (SV2A), which is involved in the release of messenger substances. This dampens the excessive signal transmission and makes the brain less prone to the abnormal discharges.
Levetiracetam is used to prevent seizures in epilepsy – both for focal seizures and for certain generalised seizure types. The active ingredient is barely metabolised in the liver and is excreted via the kidneys. That explains why it has so few interactions and combines well with other medications.
Treatment usually starts with a low dose that is increased slowly over a few weeks. A typical start is 2× 250–500 mg per day, raised step by step until the seizures are controlled – always at about the same time and independent of meals.

Levetiracetam and Lamotrigine are among the most commonly used modern anti-epileptic drugs. In everyday life, what matters most to patients is the difference in titration and side-effect profile: Levetiracetam works quickly and can be titrated up rapidly, whereas Lamotrigine has to be increased very slowly.

Which anti-epileptic drug is the right one depends on the seizure type, age, a possible pregnancy and any co-existing conditions. None is universally "the best" – the choice is made individually by the doctor.
Levetiracetam is usually well tolerated physically. The most important and most common side effects are fatigue and – often underestimated – psychological changes. Irritability, mood swings and low mood in particular are the most frequent reason patients want to stop the medication. Package leaflets often mention this only in passing – talking about it openly helps.

A major advantage of Levetiracetam: it has very few interactions, because it is barely broken down by the liver. It hardly affects other medications and is itself hardly affected. Even so, it's worth looking at a few combinations – check them in the interaction check.

Levetiracetam must never be stopped abruptly. Suddenly leaving it out can trigger clustered or severe seizures, up to the life-threatening status epilepticus. If the medication is to be discontinued or switched, the dose is reduced slowly over weeks – always according to a medical plan.
